This form provides information as to where your department will be storing your vehicle when not in use. The form is required for all vehicles leased or loaned from Fleet Services.

Driver’s Report of Accident – PDF
This is the form found in the Accident Reporting Kit, used to gather details of an accident involving a county vehicle.
Occurrence Report – LINK
This form is used to document important facts surrounding an incident or accident involving a member of the public. This could include, but is not limited to a vehicle collision, injury of a member of the public at a County facility, or an event that could possibly cause a member of the public to file a claim against the County. Note: This form does not replace the “Driver’s Report of Accident”; it may be used as a supplement to gather additional information after the “Driver’s Report of Accident” has been filled out.
