This form will allow you to submit your comments, good or bad, regarding the conduct of a county vehicle. The information will be forwarded to the head of the agency or department that the vehicle is registered to.



If you have been in an accident with a County vehicle and are seeking information or filing a claim, you and or your insurance carrier may contact the County Risk Management Department at (805) 654 – 3197 or review information at the Risk Management Website.
